Home health aides are generally used when a person says, "they just need a little help" so there's no need to hire a nurse unless labs, injections, therapy ect. have to be done. The problem here is that home health aides do not work out of an electronic medical record system from the company they work for. This means that home health aides do not have an electronic medical record that documents the day to day care of an individual for the purposes of maintaining an overall assessment of the patient and duties of the health aide; that is useful and will assist with the transition of care from aide to nurse and from nurse to physician and to keep a log as to remind the patient and home health aide of what he or she must dofor each visit.
Sally Sims is a home health aide. She works for Uptight Medical Staffing Solutions. Uptight Medical Staffing Solutions employs Sally or rather Ms. Sims to work with two clients during the weekday. As am employee Ms. Sims was given a user name amnd log on to log into the EMR for Uptight Medical Staffing Solutions. As a home health aide, Ms. Sims is then able to enter that days schedule and search for her name. She will see that she has Ms. Rocker from 9am to 1 pm and Mr. Bother from 2pm to 4:30pm. Ms. Sims can log into the EMR with any device that is used to access the internet. Ms. Simms then enters the Patient procedures/duties tab under the patients name and enters: CPT code T1021 for her per visit reimbursement, she does this for each patient. Uptight Medical Staffing also requires her to enter a revenue code for the different duties outside of her job as an aide. Those jobs include: food shopping, picking up pharmacy medication, washing clothes and dishes, assisting the patient with administering their medications(pill,capsule and liquid only) and general house cleaning. This is so that when clients such as Ms. Rocker and Mr. Bother receive their invoices it is understood that an extra charge will be added for certain duties done by the home health aide that are not included in the per visit reimbursement code of T1021. For that days service Ms. Sins then selects the revenue duties that describe the other chores that were performed. For Ms. Rocker she assisted with revenue 1-medications and revenue-6 general house cleaning. For Mr. Bother she assisted with medications; revenue -1 and she washed some dishes for him revenue -8 and accompanied him for a doctors visit revenue-12. With the use of her smart phone or tablet Ms. Sims can record her duties for each client as she sees them each day. Ms. Simms can also sign off on the procedures/duties note and she can add a note or anything additional that was done by her or requested, for the supervisor at Uptight Medical Staffing to review. The procedure/duties information is generated into a claim and and or invoice. The claim will be sent to the clients insurance company and once that reimbursement amount is received by Uptight Medical Staffing and posted by their billing department an invoice can be generated as a bill to send to the client for the balance of the payment due. What is most helpful is that when home health aides need to take information for clients that is given to them by the clients physician, all that information can be noted in a note beside the particular reimbursement cpt code or revenue code it corresponds to eg. Mr. Bother had a doctors appointment which was revenue-12. the doctor told him to make an appointment with the orthopedist and neurologist. Ms. Sims wanted to be sure to document what she was told to remember for her client Mr. Bother , so Ms. Sims documented in the note beside revenue-12 that Mr. Bother was instructed to follow up withy the orthopedist and neurologist.
